Julio Cesar began his professional career with  Flamengo in 1997 . By 2000, he was the first choice goalkeeper
for Flamengo and win the Campeonato Carioca four times.

In short time he  became a fan favorite for Flamengo. In January 2005 Julio Cesar was signed on a free transfer by Chievo Verona where he failed to make an appearance.
In July 2005 the Brazilian signed a  contract with Internazionale Milano. Everybody expected to be second choice goalkeeper for Inter Milan, where Francesco Toldo was in good form.
In 2005–2006 season saw him sitting on the bench throughout the entire season. At Brazil, the goalkeeper was the third option for Carlos Alberto Parreira in the 2006 World Cup.
In the next season in Seria A he won the battle with Toldo and he became Inter goalkeeper first choice.
In 2009,  IFFHS named him as the third best goalkeeper in the world, behind  Iker Casillas and Gianluigi Buffon.

Julio Cesar was also awarded the Serie A Goalkeeper of the Year title in 2009, ahead of Buffon of Juventus and Federico Marchetti of Cagliari.  He was nominated for the 2009 Ballon d’Or and he finished into 21st place, Casillas was the only other goalkeeper nominated.

In September 2007  with new coach Dunga, Julio Cesar became goalkeeper number 1 to national team and he answered the call with great performances.
Since then Julio Cesar is the holder of the national team goalkeeper and Inter’s first choice.
